Sent by one finance employee after a video call where every other participant was synthetic. Each transfer was authorised the ordinary way.

Asked at the moment of the money
Approval is intended to be requested when the payment moves or the bank details change, not when the message asking for it arrived. The decision happens where the risk is.
A person, not a mailbox
The approver would confirm they are live and present with the same short camera check that runs today, so an instruction cannot be approved by whoever is holding a stolen inbox.
Off the channel it came from
The approval is intended to travel somewhere other than the thread that produced the request, so a compromised account cannot quietly approve its own instruction.
What you can use today
Human verification is live and priced publicly: a short camera check that confirms a real, live person is on the other end, with prompts drawn at random for every session. It is the first pillar of the same platform, and the piece this product would be built on.
